Steps that needs to be taken by each player
1. Login to Sports Illustrated Play and Make $500 Deposit CLICK HERE
2. Complete the Following Player Documents and hand them out on Signing Day
a. 1 head shotphoto of your child. Use your smart phone.
b. Photocopy of Birth Certificate (1 copy for new players – not necessary for returning players)
c. completed US Club soccer medical form DOWNLOAD HERE.
A head shot photo, copy of the players Birth Certificate, CSC Medical Release Form and US Club Soccer Medical Form need to be scanned and given to your Coach/ Manager.

The Clarkstown has different pricing for different age groups and programs. The below pricing is for our Travel teams only. Teams are asked to do additional fundraising to be able to attend more tournaments.

Your fee's cover one Tournament Registration Fee. Any additional tournaments that your team wish to attend must be covered from the team account for which you pay into/fundraise for.
